-To register, please complete the two forms below and return them to the school nurse by May 21st.
-No appointment necessary. Students will be called to the nurse's office from their classroom.
-This clinic is not available for full-time remote students.
-School Health Link will contact parents after paperwork has been turned in.
-No cash payment will be accepted. Please make checks payable to School Health Link.
-Although School Health Link is the provider of this clinic, students may get their physical and vaccinations with their primary care physician or at Community Health Care (563-336-3000).
