Preferred First Name Change (No change to legal name)

Preferred name is the name which will display on your Google account profile/email name. A change to preferred name requires no documentation. Some common instances when someone would have a preferred name change:

Someone prefers to be referred to by their middle name
Some prefers to be referred to by a first name other than the name listed on their social security card
Someone wishes to update their district email address to reflect the first initial of their preferred name

Legal name change

Official name changes, which will be reflected on paychecks, tax forms, benefits, etc. will require employee to bring the following to the HR Department at the Administration Building, in person:

Updated Social Security card reflecting your new name
Updated state W-4 form (Illinois) (Iowa)
Updated federal W-4 form

Once you have provided us your updated social security card/work authorization, the Human Resources department will make necessary updates in Skyward Finance and to your benefits.

You will receive an email notification with new login information reflecting your new legal name after this process has been completed. You will receive a new badge via district mail.
